ASHE Region 6 Conference Sponsorship Opportunity Agreement

10th Annual ASHE Region 6 Education Conference
April 7–8, 2011 — Verizon Wireless Center, Mankato, Minn.

The American Society for Healthcare Engineering (ASHE) of the American Hospital Association (AHA), and health care engineering groups from Iowa, Minnesota, North Dakota, South Dakota and Wisconsin (ASHE Region 6) invite you to participate as a sponsor for the 10th Annual ASHE Region 6 Educational Conference, Thursday & Friday, April 7-8, 2011, at the Verizon Wireless Center in Mankato, Minn.

Volunteers from Region 6 chapters have put together an educational program that brings nationally recognized experts together to present and answer questions on the current "hot > topics" critical to health care facility managers/engineers.

Your participation as a conference sponsor will increase your visibility among key decision makers responsible for purchasing products and services vital to health care facility management and operations.

Please make every effort to support this educational conference. Your sponsorship commitment will help us develop programs for future years.

Audience/Attendees:
More than 250 area facility managers, project managers, engineering/maintenance personnel, administrators, safety officers, risk managers, architects, engineers, or other interested parties involved with facility, construction management, property management or the operation of a health care facility or public building will be attending this conference.

Sponsorship Details:
The fee for this sponsorship opportunity is $500. In appreciation for your support, your company will receive the following:

• One attendee registration for the Region 6 Conference
  • We will send you a promotional code and link as soon as conference registration is open.
• Verbal recognition of your participation at the start of the Region 6 Conference
• A listing of your company name in the conference resource manual*
• One full-page ad in the conference resource manual*
• Signage with all sponsors' names will be located throughout the conference
• Copy of the conference resource manual
• A final attendee list
• Two passes for the Networking Reception taking place on Thursday, April 7, from 4:30 to 7:00 p.m.
• 4-foot by 3-foot table space to display literature and giveaways during the Networking Reception
  • This is NOT a technical vendor show with booths.
  • If you want table-top space, please indicate so on the sponsorship form.

*To guarantee your company's acknowledgment in the Conference Resource Manual and on conference signage, sponsorships must be received by March 1, 2011.

Guidelines

• Payment is required in full with submission of this form/completion of online registration.
• Advertising Contract and Application: Confirmation will be returned to your company upon receipt of application and payment. Reservations will not be held without payment. No sponsorship or advertising benefits will be given without full payment.
• Cancellation: Your company may not cancel the ASHE Region 6 Conference Sponsorship Opportunity Agreement after its acceptance by ASHE.
• Giveaways: Any giveaways must be preapproved by ASHE. Request for approval must be submitted in writing and include a description of the item, proposed verbiage, and logo(s).
• Deadlines: Benefits of ASHE Region 6 Conference are subject to print and name submission deadlines.
• Transfer of Benefits: Benefits are not transferable and are not redeemable for cash credits.
• Limited Liability: The liability of ASHE for any act, error, or omission for which it may be legally responsible shall not exceed the cost of any cash payment. ASHE will not, in any event, be liable for consequential damages, including but not limited to, lost income or profits.
• Change, Cancellation, or Substitution of Benefits: ASHE reserves the right to change, cancel, or substitute any ASHE Region 6 Conference Sponsorship and On-Site Advertising opportunity or benefit at any time.
  

The agreement and this document represent the entire understanding and contract between parties involved and shall be governed by the laws of the State of Illinois. Any person executing this agreement represents that he or she is fully authorized to do so and agrees to be bound to all terms and conditions by signing the ASHE Region 6 Conference Sponsorship and On-Site Advertising application.

Yes, I agree to participate as a contributing sponsor for the 10th Annual ASHE Region 6 Education Conference organized and supported by the American Society for Healthcare Engineering (ASHE) of the American Hospital Association (AHA) and health care engineering chapters from Iowa, Minnesota, North Dakota, South Dakota, and Wisconsin (ASHE Region 6).

I understand that upon ASHE's receipt of this sponsorship form and payment of $500 by March 1, 2011, my sponsorship of the program will be guaranteed and my full-page ad will be placed in the conference resource manual.
